Received first packet of info from America World

Completed and mailed client agreement, client’s rights and responsibilities, Grievance Procedure and Photo release form.
January 31, 2005
Mailed I-600A form along with copies of birth certificates, marriage license.
Talked with Carla Proctor (Social worker with Jewish Family Services) on phone. Set up first meeting for February 14, 2005 at 9:00 a.m. at her office. Sent Ms. Proctor a release to get our info from MS child abuse registry.

I-600A form returned for being incomplete. We did not mark one box on #7. Fixed form and returned same day in mail.

Second home study visit with Carla Proctor

Several documents for the dossier were notarizied: Application letter to the China Center of Adoption Affairs (CCAA), Financial Statement, and Lori's non-employment verification letter.
Rick and Lori had health physicals. Waiting on the results of blood work.
February 25, 2005
Rick and Lori went to police station for notarized police reports. (Neither one has a police record!)
We also received state certification of Lori's Ohio birth certificate and Rick and Lori's Florida marriage license. We hit a glitch. Rick's birth certificate was not in the correct form for state certification. We had to order another one with a "pen in hand" signature. The new birth certificate was again sent to the Georgia Secretary of State.
February 28, 2005
Rick's employment verification letter was notarized.
Rick and Lori go to the county health department for a TB skin test.
March 2, 2005
Doctor signs off on physical form. Rick and Lori have a clean bill of health! Neither one has HIV, Hep B, or TB. Physical form is notarized!
March 3, 2005
Rick's birth certificate returns with Georgia State certification.
March 9, 2005
Carla Proctor comes for home visit.
March 15, 2005
Lori's birth certificate arrives from the Chinese Consulate in New York. Our first dossier document that has gone through all the stages! It is ready to go to China. Now we just need to finish our home study, be fingerprinted by the FBI and then receive our I-171H form. We will then have all the documents which will have to be state certified and authenticated at the Chinese Consulate. All other documents will go the the Houston Consulate.
